Privacy and Confidentiality
The Personal Health Information Protection Act (PHIPA) became law in November 2004. This privacy legislation governs the manner in which personal health information is collected, used and disclosed within the health care system as well as confirming a patient’s right to access their own health information.

Unless you tell us differently, we will let visitors or callers know your location in the hospital and your general health status. If you do not want anyone to know you are in hospital, please let us know and we can put a Confidentiality Status in the registration system.

Our commitment to you is to protect the privacy and confidentiality of your health information. Your health information will only be used in the following circumstances in accordance with our privacy program:

For research, to educate and collect statistics

And to comply with legal and regulatory requirements

Freedom of Information
Sensenbrenner Hospital is subject to the Freedom of Information and Protection of Privacy Act (FIFFA). This Act provides the public with a right of access to general records held by Sensenbrenner Hospital that are not considered personal health information.

In order to make this type of request, it must be in writing and be accompanied by a $5.00 application fee.

Accessing your Health Information
The role of the health records department is to keep an accurate legal record of your visits to the hospital. You may obtain access to your health record by submitting a release of information request form that you can download and fax, mail or drop off to us once completed.
​
Depending on the reason for the request there can be a fee attached to the request so you can call ahead to find out what the cost may be.
